Prognostic Analysis of Treatment Modalities in HCC with Portal Vein Tumor Thrombus

چکیده
Hepatocellular carcinoma (HCC) is one of the most common malignancies worldwide, especially in Asian countries. HCC has a propensity for invading the portal venous system, with an incidence of portal venous tumor thrombi (PVTT) in 62% of autopsies and 14% of surgically treated patients PVTT indicates an advanced stage of the disease and is associated with risk of esophageal variceal bleeding and liver failure. PVTT is the key factor in intrahepatic invasion and prognostic. L lovet reported a median survival time of 2.7 months for HCC patients with PVTT. Hepatic resection, liver transplantation and transcatheter arterial chemoembolization (TACE) are the common treatment in clinical practice. Some study reported that the postoperative survival rate became increased year by year for the patients who undergo hepatic resection or TACE or liver transplantation, respectively. But the most effective treatment strategy for HCC with PVTT remains to be established. The aim of the present retrospective study was to investigate postoperative prognostic factor and to propose a surgical treatment strategy for the HCC patients with PVTT.

FDG-avid portal vein tumor thrombosis from hepatocellular carcinoma in contrast-enhanced FDG PET/CT
Objective(s): In this study, we aimed to describe the characteristics of portal vein tumor thrombosis (PVTT), complicating hepatocellular carcinoma (HCC) in contrast-enhanced FDG PET/CT scan. Methods: In this retrospective study, 9 HCC patients with FDG-avid PVTT were diagnosed by contrast-enhanced fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT), which is a comb...
متن کاملA prognosis model for patients with hepatocellular carcinoma and portal vein tumor thrombus following hepatic resection
The present study aimed to identify the risk factors influencing the survival of patients with hepatocellular carcinoma (HCC) affected by portal vein tumor thrombus (PVTT), following hepatic resection, and to establish a prognostic model. Between March 2001 and May 2008, 234 cases of HCC with PVTT that underwent hepatic resection were randomly divided into experimental or validation groups. The...
متن کاملHepatocellular carcinoma patients may benefit from left hemi-hepatectomy with caudate lobectomy
In order to archive a better prognosis, left hemihepatectomy is usually used as the first-line treatment for the hepatocellular carcinoma (HCC) patients with left portal vein tumor thrombus. Because of the short distance between hilar bile duct and caudate lobe bile duct, hilar cholangiocarcinoma has a high metastasis rate on caudate lobe ducts. Caudate lobectomy contributes to improvement of d...
متن کاملMultidisciplinary management of hepatocellular carcinoma with portal vein tumor thrombus – Eastern Hepatobiliary Surgical Hospital consensus statement
Hepatocellular carcinoma (HCC) complicated by portal vein tumor thrombus (PVTT) is associated with poor prognosis, early recurrence of HCC, and limited treatment options. Current guidelines do not have standardized diagnostic and treatment modalities, thus creating a need for a multidisciplinary treatment model for standardization of the treatment. Eastern Hepatobiliary Surgical Hospital (China...

UNLABELLED Hepatocellular carcinoma (HCC) is one of the leading causes of cancer-related death. Portal vein tumor thrombus (PVTT) is one of the most dreadful complications of HCC and is associated with a median survival time of 2.7-4.0 months. The optimal treatment for HCC with PVTT has not yet been established. The aim of the present study was to report long-term results of percutaneous radiof...
